Acknowledgement of Risks
I understand that playing hockey on the lake is not without risk of personal injury, including serious injury and death, which may be caused by falling on or through the ice.
Assumption of Risks
In order to participate in the North American Cup (Feb21-23th, 2025) I agree to assume any and all risks associated with my participation in this activity. I understand that my participation in this activity may lead to loss or damage to personal property, personal injury, including serious injury and death and I accept these possibilities in order to participate in this activity.
Release of Liability and Waiver of Claims
I agree to release Rob Carleton & On The Pond! Canada, Gravenhurst Boston Pizza, the Town of Gravenhurst and its affiliated and associated companies, its owners, managers, employees, volunteers, from any and all liabilities, including negligence that may result from my participation in this pond hockey tournament. I further agree to waive all claims that may be brought against On The Pond! now, or at any time in the future, by me, my estate, heirs, executives, representatives, relatives, or any other party acting on my behalf.
I have read and understood this participant Acknowledgement and Assumption of Risk, Release of Liability, and Waiver of claims Agreement (hereinafter referred to as the “Agreement”). I further understand that I am forfeiting valuable personal and legal rights by signing this Agreement and I am basing my decision to enter into this agreement solely on the information provided within this document. Furthermore, I agree that this Agreement shall be binding on me, my estate, heirs, executives, representatives, relatives, or any other party acting on my behalf.
